This is where you can see and download the all the entries made to your templates.

Entry Fields

Document UID

This is a unique identifier the system has applied to this entry, and identifies the Document created. This can be is used to track documents that have been faxed or mailed when created from one of your templates using the DocumentUID Dynamic Element Code.

Date of entry

This is the date that this entry was created, using the Time Zone set in your account settings.

Date modified

This is the date that this entry was last modified, using the Time Zone set in your account settings. This will match the "Date of Entry" unless the entry has been modified. If you make a modification to the entry using the "edit" button, this date will change

IP address

This is the IP Address of the user who completed your form.

Custom1 and Custom2

These are custom values that you can set in the URL of your template when you display it to a user.

You can use any value up to 50 characters long within these fields. You can use it for a wide range of values, though the main use is to associate a userID from your own website/application with a Doculicious entry - thus allowing you to match up content in your Doculicious account to a user from your own website. To do this, you would just add &custom1=xxx to the URL you embed on your website or display to the user, where xxx is the user ID assigned by your application. This would make your embed URL look similar to this:


You can then search on and filter your entries using this value in both the entries screen and the entries API.

This small addition to Doculicious brings a very powerful feature that you can use to enhance your own website and give your users more value.

Some ideas of what you could use this value for:

  • Your applications user ID
  • A TRUE/FALSE flag to indicate something for your application, ie: a valid purchase.

Admin comment

This field is editable and allows you to write any type of comment associated with this entry. Commonly used to annotate orders or applications. 


This check box can be used for you to flag the application as processed or not, and gives you the ability to filter the entries. Commonly used for sales or applications so that you can clear ones you have completed.
